Rooted is a four-week campaign designed to return participants to the basics of well-being.

Participants are encouraged to reconnect with simple sustainable habits and to discover what matters most.
With an intentional and grounded approach to each day, Rooted can empower individuals to build lasting habits for better physical and mental health.

1. REGISTER ONLINE OR ON-SITE

Register by May 29 online or on-site at the Eagan Center for Wellness.


2. ATTEND KICKOFF WEEK

Stop by during kickoff week, May 20-29, at the Eagan Center for Wellness to receive your campaign tracker and t-shirt (while supplies last).


3. DISCOVER WHAT MATTERS MOST

Each day, write the number of the activity you completed on a leaf on your tree. Choose at least four per week but make it a goal to complete your tree. Feel free to include your own activities. As an alternative, you may track your activities through the Survey Monkey links that will be emailed weekly.


4. SUBMIT YOUR TRACKER
Submit your campaign tracker to the Eagan Center for Wellness or document your activities using the emailed survey links by July 6 for campaign credit.
